Finding a CPSO-approved supervisor in Ontario

You passed the exam. You have the training. And you cannot start, because a restricted certificate needs a supervising physician and there is no list of them to apply to. This is the step that stalls more internationally trained specialists than any other — and the one thing on this site we do rather than explain.

Why this is the wall, and not just another step

Almost every obstacle on the way to Canadian practice is a process. Verification has a form. Exams have dates. Applications have fees and timelines. You can read the requirements, do the work, and move forward.

Supervision is not a process. It is a person deciding to put their professional name alongside yours. There is no register of physicians offering to supervise, no application form, and no queue to join. Qualifying supervisors are limited, already busy, and have no reason to advertise — which means the physicians who find one are, overwhelmingly, the ones who already knew somebody.

That is why preparation does not solve it. You can be entirely ready and still be stopped, and the stopping is invisible from the outside — it looks like nothing is happening, because nothing is.

We connect Royal College PER-eligible specialists with experienced Ontario physicians willing to supervise. This is the part that is genuinely hard to do alone, because it depends on relationships rather than on any published process.

A supervisor is agreeing to put their name to your practice. We help you present your training record the way a Canadian department reads it, so the first conversation starts from the right place.

We stay with you through supervision

Supervised practice has requirements, reporting and a defined end point. We guide you through what the College expects during that period and how it leads toward an unrestricted certificate.

Supervisor matching is currently Ontario only, through the College of Physicians and Surgeons of Ontario. Supervision requirements differ by province, and we would rather do one properly than four badly. If you are targeting another province, start the eligibility check anyway — the assessment covers every province, and we will tell you where you stand.

What we cannot do: guarantee a match, or influence what the College decides. Whether a supervisor is acceptable for your certificate is CPSO’s determination and depends on your file. Our clinical lead practises in Ontario and directs a Canadian fellowship — if any supervisor we introduce you to is connected to MedPathways, we tell you before the introduction, not after.

Questions physicians ask about supervision

Why do internationally trained specialists need a supervisor in Ontario?
Certification and licensure are separate. Passing the Royal College examination addresses certification; practising in Ontario requires a certificate of registration from the College of Physicians and Surgeons of Ontario. Where that certificate is a restricted one, it carries a requirement to practise under a supervising physician acceptable to the College. Supervision is therefore not an optional extra step — for many specialists it is the mechanism by which they are permitted to practise at all.
Why is finding a supervisor so difficult?
Because it is not a process, it is a person. There is no register of physicians offering supervision and no application form for being matched with one. A supervisor is agreeing to attach their professional name to another physician’s practice, which is a serious commitment made on the basis of trust and familiarity. Qualifying supervisors are limited in number, already busy, and rarely advertising. A physician with no Canadian network has almost no way in.
How long does supervised practice last?
For Royal College certification through the Practice Eligibility Route, the Guidebook sets a minimum of two years of supervised practice in Canada, in a continuous practice location, within the primary specialty, with at least 50% of practice in that specialty. Separately, the duration and conditions of any restricted certificate of registration are set by the provincial College and depend on your file. These are two different clocks set by two different bodies, and they need to be tracked together.
Can time in a Canadian clinical fellowship count toward the supervised practice requirement?
In some cases. The Royal College PER Guidebook allows one year of Canadian clinical fellowship to count toward the two-year practice requirement where the fellowship significantly overlapped the primary specialty and at least 50% of the time was dedicated to it, attested by the fellowship director. The important restriction: fellowship time already counted toward your training requirements cannot also be counted toward practice. This is the single most common misunderstanding we see.

We do not guarantee a match, and whether a particular supervisor is acceptable for your certificate is the College’s decision, not ours.
Do you help with supervision outside Ontario?
Not currently. Supervisor matching is Ontario only, through CPSO. Supervision requirements differ meaningfully by province and we would rather do one properly than four badly. Our pathway assessment covers every province, so if you are targeting another jurisdiction the assessment is still the right starting point.

Registration requirements, certificate classes and supervision conditions are set by the College of Physicians and Surgeons of Ontario and can change. Confirm what applies to your own file at cpso.on.ca. The Royal College sets certification requirements separately, including the supervised practice period on the Practice Eligibility Route.
