Does Royal College certification let me practise medicine in Canada?

No. Certification and licensure are separate, and they are granted by different bodies. The Royal College of Physicians and Surgeons of Canada certifies specialists; the licence that actually permits practice is issued by the College in the province where you intend to work. You can hold certification and still not be licensed to practise.

What is the biggest obstacle for internationally trained physicians coming to Canada?

For most physicians it is supervision, not examinations. Provisional and restricted licence classes require a sponsoring employer and a supervisor who meets the provincial College standards. Qualifying supervisors are limited, regulated and in high demand, so this is usually the step that stalls a file — and it is largely outside the applicant’s control.

Is Ontario the fastest province for an internationally trained specialist to get licensed?

Often not. The College of Physicians and Surgeons of Ontario is comparatively restrictive and generally expects certification for independent practice. Saskatchewan, Alberta and the Atlantic provinces run structured supervised-entry routes that can be considerably faster. Ontario is frequently better treated as a destination than as the first door.

What is the Practice Eligibility Route (PER)?

PER is a Royal College route that allows experienced specialists trained outside Canada to be assessed for eligibility to sit the certification examination, generally where their postgraduate training is comparable to a Canadian programme. It is important to understand what PER grants: access to the examination process, not certification itself and not a provincial licence.

Do I need Canadian work authorization before I start the licensure process?

Work authorization and licensure are separate processes, but a provincial College will not issue a licence permitting practice to someone who cannot legally work in Canada, and most sponsoring employers require status to be resolved before they will commit. In practice the two need to progress together rather than one after the other. MedPathways does not provide immigration advice — in Canada only lawyers, Quebec notaries and CICC-licensed consultants may do that.

How long does credential verification through the Medical Council of Canada take?

Source verification of medical credentials through physiciansapply.ca typically runs on the order of months and depends on how quickly your issuing institutions respond. It sits on the critical path of every licensure route, so starting it early is one of the few things that shortens every pathway at once rather than just one.

Is MedPathways immigration advice?

No. MedPathways provides guidance on medical licensure and the routes to Canadian practice. It does not provide immigration advice or representation, which in Canada may only be given by lawyers, Quebec notaries, and consultants licensed by the College of Immigration and Citizenship Consultants. Where work authorization is the obstacle, we can introduce you to a licensed immigration lawyer, but the advice itself comes from them.

Which specialties does MedPathways cover?

Internal medicine and medicine subspecialties — cardiology, nephrology, respirology, gastroenterology, endocrinology, rheumatology, infectious diseases and the rest of the general internal medicine family. Other specialties, and family medicine, are outside what we assess. We say so at the eligibility stage rather than taking payment for guidance we are not the right people to give.

Can family physicians use MedPathways?

Not currently. Family medicine follows a different certification body and a different set of provincial routes, and guidance built for internal medicine would be misleading applied to it. The eligibility check will tell you this before you spend anything.

Does an assessment guarantee licensure or a residency position?

No, and any service that suggests otherwise should be treated with suspicion. Licensure decisions rest entirely with the Royal College and the provincial Colleges, and residency matching is a separate process run through CaRMS. What an assessment does is tell you which routes are realistically open to your file, what is most likely to stall you, and what order to do things in.

What if I trained in India, Pakistan, Nigeria or another country without an established Canadian integration route?

It changes the route, not whether there is one. Boards from the UK, US, Australia and Ireland have more established recognition arrangements, so physicians holding those often move faster. Everyone else generally leans harder on the Practice Eligibility Route or on provincial supervised-entry classes, where what is assessed is the depth and comparability of your documented training rather than which board issued your certificate. Credential source verification through the Medical Council of Canada also tends to take longer, which is a scheduling problem rather than a barrier.

Egyptian medical training is university-based and academically demanding, and Egyptian physicians form one of the larger internationally trained groups seeking Canadian practice.

The thing worth knowing first

Canadian assessment is interested in the substance of your postgraduate training — how long it lasted, what it contained, and who supervised it — rather than in which body issued your certificate. Two physicians with the same training and different qualifications on paper are in more similar positions than either usually believes.

How your training reads to a Canadian assessor

Egyptian postgraduate training typically runs through the universities — an MSc followed in many cases by an MD, alongside clinical residency. The academic and clinical components are somewhat more separable than in systems built around a single college fellowship, which sometimes causes confusion in assessment.

What matters is showing the clinical training clearly: the years spent in supervised practice, the rotations, the responsibilities held. Academic degrees are valuable, but comparability assessment is principally interested in the clinical training that ran alongside them.
